Job Description

Position Summary
The Enhanced Supportive Housing Program (ESH) Nurse provides the medical and psychiatric monitoring and direct care as appropriate to state laws and as prescribed by a MD. These services are designed to support program participants with diagnosis of serious mental illness living in the community to improve/maintain their quality life and their housing. This position is requires work in both Raleigh and Durham.
Responsibilities include: monitoring medical and psychiatric status of the consumers in collaboration with the physician, medication instruction, symptom management, activities and interventions to enhance well-being.
Essential Duties and Functions:
  • Provide injections for individuals in the program as monthly and as needed
  • Assist with monitoring for changes in emotional and physical health
  • Review Medications as appropriate 
  • Ensure appropriate medications are available and documented accurately.
  • In collaboration with the consumer and physician formulate a service plan designed to promote good health practices and participation in the recovery process.
  • Assist in obtaining and/or interpreting medical information to improve the team member's knowledge base regarding consumers' health status, needs, and strengths.
  • Insures MD orders are carried out accurately and promptly.
  • Consults with team regarding high risk, at risk program participants.
  • Participates in specialty training's for team members and consumers.
  • Document in electronic health record.
Other Duties:
  • Work in a collaborative manner with other staff, families and provider agencies in assisting the consumer and the program to meet identified goals.
  • Participate in on-going planning and the development and implementation of a Continuous Quality Improvement program.
  • Perform other duties as assigned within the general functions of the position.

  • High School diploma/GED
  • Must have successfully completed an accredited LPN program and posses a current license to practice in the state of North Carolina.  
  • Must have a minimum of 2 years previous experience plus an understanding of and interest in working with persons with serious mental illness in the home environment. 
  • Must be willing to work in both Raleigh and Durham
  • Valid driver's license is required.
