Job Description

THIS POSITION IS IN THE LEHIGH VALLEY AREA, APPROXIMATELY 50 MILES NORTH OF PHILADELPHIA.

The On Our Own II program is looking for Direct Support Professional to work in Palmerton, PA.

On Our Own II is a community-based residential program that serves adults diagnosed with Intellectual and Developmental Disabilities, Autism, and other mental health challenges. Direct Support Professional are responsible for working in accordance with the values and policies of RHD, as well as, the state regulatory guidelines. Direct Support Professional will provide direct care to individuals served by assisting with medical appointments, treating all individuals with dignity and respect, implementing clinical programming, and maintaining accurate documentation.

Direct Support Staff are expected to promote the values and image of Resources for Human Development and to adhere to all of the agency's Policies and Procedures, at all times.

Positions include all shifts - full-time, part-time and on-call (as needed)  

**FULL-TIME $500 Sign-on Bonus offered!  Part-time $300 Sign-on Bonus! Full time* DSP will receive $250 and Part-time DSP's will receive $150 following the completion of New Hire Orientation.  Then following the 1st 90 days of successful employment, Full-time staff will receive additional $250 and Part-time staff will receive additional $150.

Essential Duties and Functions
  • Approach the individuals served, at all times, with unconditional positive regard. This includes both attitudes and actions.
  • Adhere to program schedules and arrive for all shifts, meetings, trainings, and Professional Development on time.
  • Teach, guide, and support individuals to develop skills necessary for happy, healthy, and safe interactions in their home and in the community.
  • Provide personal care, as needed, to assist individuals to maintain health and appropriate public appearance.  This may include, but is not limited to, bathing, diapering, tooth brushing, and other direct care responsibilities.
  • Physical lifting and transferring of consumers is an essential job function.
  • Engage in physical activity to meet the certification requirements of Crisis Prevention and Intervention training and consumer needs.
  • Meet the certification requirements for First Aid / CPR Training.
  • Meet the certification requirements for Medication Administration Training
  • Ensure safety of individuals, by conducting safety checks, evacuation procedures, and monthly fire drills.
  • Implement supportive interventions at all times, especially in the moment of a crisis, such as episodes of self-injurious behaviors or aggression.
  • Accompany and support individuals during community integrated activities, initiate and develop meaningful and lasting interactions with other members of the community.
  • Provide services to the individuals, in whatever setting is necessary, including:  their homes, community, hospitals, day programs, nursing homes, and family homes.
  • Locate or develop resources and activities to promote positive community involvement.
  • Perform duties associated with participation in Individual Support Plans and ensure that program planning is the primary focus within the home.  Responsible for implementation and appropriate documentation of ISP outcomes (goals) as written in the individual's ISP.  Maintain staff supervision ratios, as written in ISP.
  • Assist the team to assess current skill levels and maintain awareness of environmental factors or issues and their impact on the person(s) served.
  • Assist the team to generate ideas for programming.
  • Drive individuals to day program, medical appointments, community outings, and other destinations, in a safe manner utilizing an agency vehicle.
  • Write all documentation in a thorough, accurate, and legible manner, including, but not limited to: medication logs, individual daily logs, goal tracking, timesheets, incident reports, injury reports, motor vehicle reports, medical forms, communication logs, etc.
  • Administer all medications (OTC and prescription) and prescribed therapies, safely, accurately, and uphold regulatory standards regarding medical issues.
  • Adhere to directives and procedures developed by the Team Leader, Program Specialist, Program Nurse, and/or Program Director.
  • Interact and communicate with co-workers, supervisors, and people served in a respectful, professional, coherent, and cooperative manner.
  • Communicate and collaborate with other agencies and professionals in a respectful, professional manner.
  • Work within ALL of your individual program's homes as programmatic need dictates.
  • Successfully participate in training, staff meetings, supervisory sessions, and all other required meetings, as required, as part of your employment with RHD/ On Our Own Program.  Meetings and trainings are typically offered between the hours of 8:30 am and 5 pm.  All trainings and meetings are required as part of your employment.
  • Ensure that all areas in the home and surrounding property are clean, sanitary, safe, and well maintained by cleaning and maintaining the home on a daily basis.  Maintain safety in the home, by reporting any physical site problems immediately.
  • Manage money and receipts accurately and timely.
  • Promote and encourage ongoing interaction with individuals, as appropriate for each individual.
  • Know the individuals you work with well.  Be able to assess their needs and medical needs to be able to respond, in a responsible and timely manner, to their needs, particularly in the time of a medical situation or crisis.
  • Maintain open lines of communication with the Assistant Site Supervisor, Site Supervisor, Program Specialist, Program Nurse, and Program Director, regarding the needs of individuals served, as well as, any staffing issues or concerns.
  • Perform other related duties within the general framework of the position, as instructed by supervisory personnel.

Minimum Qualifications:
  • Candidates must possess a High School Diploma or GED and a valid driver's license.
  • Applicants with prior related work experience are preferred.
  • Knowledge of the impact of trauma on the well-being of the individuals receiving services, as well as the impact on staff well-being preferred.
