Job Description

Ocean County RIST's (Residential Intensive Support Team) goal is to support and encourage individuals who experience mental illness in the development of life skills required to sustain successful living in the community. Position Summary
The Case Manager's role is to ensure the overall wellness and recovery of servicing individuals with severe and persistent mental illness in the community.
 
Essential Duties and Functions
 
Responsibilities:
  • Determine resources needed to meet the individualized needs of identified consumers in the assessment process and coordinate community support activities.
  • To take all necessary actions in assuring the implementation of treatment plans including serving as an advocate to the consumer to enable him/her to receive needed services; identification of program needs, service gaps, and strategy recommendations.
  • To support the ongoing coordination of services and support systems to ensure continuity of intervention.
  • To maintain consumer records, review and update consumer evaluations, complete reports of services provided, services needed and all other clinical and administrative records deemed necessary and appropriate.
  • To perform home visits, meet with families, staffs of various agencies and programs and develop strategies to maintain the consumer in his/her community setting.
  • To maintain records of after care and follow up progress.
  • To attend delegated pre-discharge and discharge meetings when applicable.
  • Maintain a minimum of 24 hours of face to face (billable) time for consumers per week (subject to change by supervisor).
Documentation Responsibilities:
  • Documents all interactions with consumer, collateral, and/or families.
  • Insuring all interactions are logged into EHR and notes are filed in charts in timely manner in accordance to DHS and RHD regulations.
Other Responsibilities
  • Attend Clinical Case Consultation, Treatment Plan Update meetings, and Clinical Supervision as scheduled.
  • Provide Crisis Intervention to consumers.
  • Assist in consumer aftercare planning and follow-up.
  • Responsible for on call rotation when assigned.
  • As assigned by Director, to ensure community of care and foster a supportive nurturing environment for growth.
  • Expected to meet monthly required benchmarks of consumer contacts.
  • Use of personal vehicle is required. May have to drive in inclement weather.
  
Qualifications
  • Bachelors in Social Work or Related field and 1 year of mental health experience.
  • Valid NJ Driver's License and vehicle insurance in good standing.
  • Consent to check background on DHS Registry of Criminal DD Offenders. Consent for NJ State Criminal Background Check.
  • Must be able to work weekends.
Salary range: $15.87 - $16.59 depending on experience.
