Job Description

Located in Germantown, Womanspace Philadelphia is a 10-bed residential facility for females with a history of chronic homelessness. WSP provides evidenced based treatment from a Recovery Model perspective.  We are seeking a full-time 8am-4pm Community Care Specialist  to work with women who have substance use treatment needs along with overlapping medical, familial and mental health needs.  

Job Duties/Responsibilities:

The full-time 8 a.m.- 4 p.m. CCS staff will work directly with our residents, supporting them with the following life building skills

  • Identify and access community resources and provide support to achieve treatment goals. 
  • Build relationships,
  • Provide encouragement,
  • Oversight/demonstration of residential cooking and cleaning skills,
  • Medication monitoring,
  • Documentation completion, 
  • Accompany individuals on various outings/appointments,
  • Schedule leisure activities in the community

CCS will also work directly with Clinicians, Psychiatrist, and outside agency staff (Case Managers, PCPs) to contribute to our holistic approach to treatment.

Appropriate candidates will be compassionate, caring and enthusiastic about our mission, and will possess the desire to interact with individuals creatively through modeling, teaching, assisting, and listening. This is a fast paced environment where communication, follow through and team work are crucial skills for all employees so that we can be successful in our service delivery. Ability to interact with everyone encountered from a Trauma Informed perspective is a must.  Computer skills are also necessary for communication, to complete documentation and navigate the Electronic Health Record. 

Job Requirements/Minimum Qualifications:

  • High school diploma or GED-  some human service related post secondary education is preferred. .
  • Valid Driver's License
  • CPR/First Aid certification is a plus.
