Job Description

School Therapeutic Services is an intensive, short-term service offered in selected schools in the Philadelphia School District. This program was developed to support students whose emotional and behavioral issues create barriers to their school success. A team of experienced mental health professionals works closely with school staff and families to ensure that students receive the help and support they need to learn and thrive in school.

STS is seeking a 10 month, full-time Care Coordinator. 

Job Summary:  
Coordinate families for authorization/re-authorization for School Therapeutic Services. Coordinate behavioral health services across home, school, and community settings.  Facilitate family connection to community resources (i.e. after school program, medical assistance, community activities etc.).

Job Responsibilities:
  • Service Coordination
  • Help families maintain medical assistance
  • Accompany family/client to appointments as necessary.
  • Maintain ongoing communication with family.
  • Attend daily referral and planning meeting as requested.
  • Schedule evaluations and MSE/ Addendums.
  • Submit authorizations packets, document missed appointments.
  • Summer Camp completion, Check EVS for schedule.
  • Follow up w/ families; schedule evaluations 45-60 days in advance.
  • Assist families w/ grievance process. 
  • Other duties as assigned.
  • Coordination of paperwork completion
  • Scheduling evaluations and family follow-up
  • Case Documentation
  • Assist with audits
  • Gather all information for evaluations.
  • Send letter for scheduled/reschedule evaluations.
  • Maintenance of Service Log
Other: 
1. Electronic Health Records Admissions data entry.
2. Complete monthly assurance calls.

Minimum Qualifications:
  • Bachelor's degree in a social services field of study
  • Two years of previous, related work experience
  • Valid driver's license
  • Child Abuse, Criminal and FBI clearances to be valid within 30 days of hire.
The pay rate for this position is $14.50 - 14.66/ hour.  A comprehensive benefits package is offered.
